Appointment
The first step towards a beautiful, healthy smile is to schedule an appointment. Please contact our office by phone or complete an online appointment request.
There will be a $50 charge for same day cancellation, missed appointment or no-show fee; which is your responsibility to pay in addition to any other fees you may owe. We understand emergencies do happen and those will be taken in consideration.
Patient Forms
If you are a new patient to our office, please visit our patient forms page to complete our online patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience, we accept cash, personal checks, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
Dental Insurance
Please carefully read our office policy and agree to the following: Payment is due prior to services being rendered. All payment plans are offered though CareCredit; please see our financing section.
Insurance Companies does not guarantee payment until the claim has been received, reviewed, and processed. We do recommend Pre-Determination or Pre-Authorization be sent to your Insurance Company prior to services being rendered. Insurance does not cover all procedures and may have frequency limitations. Please check with your Insurance Company to verify what is and is not covered as well as if our office is in your network. Any procedure not covered or paid in full by your Insurance Company will be your responsibility.
We take the following Insurance Plans and these are subject to change: Aetna DMO, Aetna PPO, Amerigroup, Ameritas, BCBS PPO, Cigna DHMO, Cigna PPO, Cigna HealthSprings, Cover Kids, Delta Dental PPO and Premier, Delta Care USA, Dentemax, Dentaquest, Guardian, Humana PPO, Humana Medicare Advantage Plan, MetLife, Tenncare, United Health Care, United Health Care Dual Complete, United Concordia PPO, United Concordia ADDP.
Collections: The patient or guarantor is responsible for the entire cost of treatment received. If your financial obligations are not met, you will be sent to collections unless prior arrangements for payment have been made with the office. Once it is sent over to collections, we will not be able to take any additional payment on the account.
